Basophils

The rarest type of white blood cell, reported as part of the full blood count differential. Basophils are involved in allergic reactions and inflammation. Their number is normally very low, so the main point of interest is a persistently raised count, which can occasionally be a clue to certain blood disorders.

What it measures

Basophils are the least numerous of the white blood cells and are counted as part of the differential — the breakdown of the total white cell count into its types (neutrophils, lymphocytes, monocytes, eosinophils and basophils). They are part of the immune system's response to allergy and inflammation: basophils contain granules of histamine and other chemicals that they release during allergic reactions, and they share some functions with the mast cells found in tissues. Because their normal number in the blood is so low, a low or zero count is not clinically meaningful and cannot be reliably measured. The count is therefore mostly of interest when it is raised (basophilia). A modest rise can accompany allergic conditions, chronic inflammation such as inflammatory bowel disease or rheumatoid arthritis, an underactive thyroid, and some infections. More importantly, a persistent, unexplained basophilia can be a clue to a disorder of the bone marrow — in particular chronic myeloid leukaemia and related conditions — so a genuinely and repeatedly high basophil count, especially with other abnormalities in the blood count, is something a doctor will want to look into. As with all the differential counts, basophils are interpreted alongside the total white cell count and the other cell types rather than in isolation.

What a high value can mean

  • Allergic and inflammatory conditions — allergy, chronic inflammation such as inflammatory bowel disease.
  • Chronic myeloid leukaemia and related myeloproliferative disorders — a persistent, unexplained basophilia is an important clue.
  • Underactive thyroid (hypothyroidism).
  • Some infections — particularly certain viral infections.

  • A low or absent basophil count is normal and not clinically meaningful — their baseline number is already very low.

Basophils are read as one part of the white-cell differential, not on their own. A mild, one-off rise is common and usually reflects an allergic or inflammatory process and needs no action beyond addressing the cause. What matters more is a basophil count that is persistently and unexplainedly raised, especially alongside other changes in the full blood count — this can be a clue to a bone-marrow disorder such as chronic myeloid leukaemia and would prompt a doctor to investigate further.
