Factor VIII

One of the key proteins in the blood-clotting cascade. It matters at both extremes: a low level causes haemophilia A, the classic bleeding disorder, while a persistently high level is a recognised risk factor for abnormal clots. It is measured as a percentage of normal activity.

What it measures

Factor VIII is one of the clotting factors — proteins that work in sequence to form a stable blood clot. It measures as a percentage of normal clotting activity, and both a low and a high level are clinically meaningful. A low factor VIII is the cause of haemophilia A, an inherited bleeding disorder (usually affecting males) in which clotting is impaired, causing bleeding into joints and muscles, prolonged bleeding after injury or surgery, and easy bruising; the level roughly tracks severity. A low factor VIII also occurs in von Willebrand disease, because von Willebrand factor normally carries and protects factor VIII in the blood. At the other end, a persistently high factor VIII is one of the more common contributors to a clotting tendency (thrombophilia): high levels increase the risk of venous clots such as DVT and pulmonary embolism. Factor VIII also rises temporarily as an acute-phase reactant during inflammation, illness, pregnancy and exercise, so a single high reading is interpreted with care. It is measured to diagnose and monitor haemophilia A, to help work up von Willebrand disease, and as part of a thrombophilia investigation.

What a high value can mean

  • A thrombophilia (clotting) risk factor — a persistently high level increases the risk of venous clots (DVT, pulmonary embolism).
  • Acute-phase rise — inflammation, infection, illness, surgery, pregnancy and exercise raise it temporarily.
  • Older age and certain blood groups — associated with higher baseline levels.
  • Interpreted with a repeat when raised in illness — to separate a temporary rise from a true elevation.

What a low value can mean

  • Haemophilia A — an inherited deficiency causing a bleeding disorder; the level tracks severity.
  • Von Willebrand disease — low von Willebrand factor lowers factor VIII, which it normally carries.
  • An acquired factor VIII inhibitor — an antibody against factor VIII (rare but serious).
  • Prolonged bleeding after injury or surgery, joint and muscle bleeds, easy bruising — the clinical effects.

Factor VIII is a specialist coagulation test, ordered to diagnose and monitor haemophilia A, to help characterise von Willebrand disease, or as part of a thrombophilia work-up when a persistently high level is suspected of contributing to clots. A low level causing a bleeding tendency is managed by haematology, sometimes with factor replacement, and needs care around surgery, dental work and injuries.
