Monocytes

A white blood cell that patrols the blood and matures into a macrophage in the tissues, clearing microbes and debris. A modestly raised count is common during recovery from infection or in chronic inflammation, and is usually a minor part of the blood-count picture.

What it measures

Monocytes are the largest white blood cells. They circulate briefly in the blood before moving into tissues to become macrophages and dendritic cells — the cleanup and surveillance crew of the immune system, engulfing microbes and dead cells and helping coordinate the immune response. They form a small proportion of the white cells. A raised monocyte count (monocytosis) is often a non-specific, minor finding — common during the recovery phase of an infection, and with chronic infections (such as tuberculosis), chronic inflammatory and autoimmune conditions, and recovery of the bone marrow after suppression. A persistently and markedly raised monocyte count, especially in an older adult, occasionally points to a bone-marrow condition and is looked at more closely. A low monocyte count is usually not significant on its own.

What a high value can mean

  • Recovery from an infection — a common, benign cause.
  • Chronic infections — such as tuberculosis or endocarditis.
  • Chronic inflammatory and autoimmune diseases — e.g. inflammatory bowel disease.
  • Bone-marrow recovery — after chemotherapy or suppression.
  • Rarely — a bone-marrow disorder (such as chronic myelomonocytic leukaemia) with a persistent, marked rise, usually in older adults.

  • Usually not significant — can occur with acute stress, steroids, or after chemotherapy affecting the marrow.

A mildly raised monocyte count is usually a minor, non-specific finding — often simply reflecting recent or ongoing infection or inflammation — and is interpreted within the whole blood count rather than acted on alone. It becomes more relevant when it is persistently and markedly raised, especially in an older adult or alongside other abnormal blood counts, when it prompts a closer look for a chronic infection or a bone-marrow condition.
