Oxidized LDL (ox-LDL)

LDL cholesterol particles that have been chemically altered by oxidation, making them especially good at inflaming the artery wall and building plaque. A largely research and specialist marker, it reflects the oxidative, inflammatory side of the process behind heart attacks and strokes.

What it measures

Oxidized LDL is LDL ('bad cholesterol') that has been chemically modified by oxidation — damaged by reactive molecules within the artery wall and bloodstream. This modification matters because oxidized LDL is much more atherogenic than ordinary LDL: it is readily taken up by scavenger immune cells to form the 'foam cells' at the heart of a plaque, and it actively drives inflammation in the artery wall, a central mechanism in atherosclerosis. Measuring it aims to capture not just how much LDL is present but how much of it is in this damaged, inflammatory form, offering a window onto the oxidative-inflammatory side of cardiovascular disease. Levels tend to be higher in established coronary artery disease, metabolic syndrome, diabetes and with smoking and oxidative stress, and it has been studied as a marker of risk and of plaque activity. In practice, oxidized LDL remains largely a research and specialist test rather than a routine one: assays are not standardised, reference ranges vary, and it is not yet part of everyday risk scoring. Oxidized LDL is largely a research and specialist marker rather than a routine test, and it is not yet part of standard cardiovascular risk scoring. Where measured, a raised level reflects a greater oxidative and inflammatory burden on the arteries — the damaged, plaque-building form of LDL — and tends to accompany established heart disease, metabolic syndrome, diabetes and smoking.
