Prothrombin time (PT)

A timed test of how quickly your blood forms a clot through one of its two main pathways. A longer time means clotting is slower. It is the basis of the INR used to monitor the blood-thinner warfarin, and is also a sensitive early sign of liver disease and vitamin K deficiency.

What it measures

The prothrombin time measures, in seconds, how long a sample of your plasma takes to form a clot when a clotting trigger (tissue factor) is added. Blood clotting works through a cascade of proteins called clotting factors, and the PT specifically tests the 'extrinsic' and 'common' parts of that cascade — the factors I, II (prothrombin), V, VII and X. Several of these (II, VII, IX and X) depend on vitamin K to be made in the liver, which is why the PT is so useful clinically. It has three main uses. First, and most commonly, it is the basis of the INR — a standardised version of the PT used to monitor people taking the blood-thinner warfarin, keeping their dose in a safe range. Second, because the liver makes most clotting factors, a prolonged PT is a sensitive early marker of significant liver disease and part of assessing its severity. Third, it detects vitamin K deficiency, since that vitamin is needed for several PT factors. A prolonged PT therefore signals slower clotting and, depending on context, points to anticoagulant treatment, a liver problem, low vitamin K, or a factor deficiency.

What a high value can mean

  • Warfarin (blood thinner) — the expected, monitored effect; tracked via the INR.
  • Liver disease — the liver makes clotting factors, so a prolonged PT is a sensitive marker of significant liver impairment.
  • Vitamin K deficiency — several PT factors depend on vitamin K.
  • Disseminated intravascular coagulation (DIC) — clotting factors are consumed.
  • Inherited deficiency of a clotting factor in the PT pathway (uncommon).

  • A normal (or shorter) PT — the expected result; clotting through this pathway is working normally.
  • A short PT is rarely clinically important and does not usually indicate a problem.

When to discuss with a doctor

For most people, the prothrombin time matters through the INR, used to keep warfarin in its safe therapeutic range — too low risks clots, too high risks bleeding — and the dose is adjusted accordingly. Outside anticoagulation, a prolonged PT that is not explained by a blood thinner is a meaningful finding: it can be an early, sensitive sign of significant liver disease, a marker of vitamin K deficiency, or, with other abnormal clotting tests, part of a serious problem such as DIC.
