Urine protein (proteinuria)

A measure of how much protein of all kinds is leaking into the urine. Healthy kidneys let very little through, so proteinuria is an important sign of kidney damage; the amount helps show how severe the problem is and is used to diagnose and monitor kidney disease.

What it measures

Normally the kidney's filters hold back the proteins in the blood, so only a tiny amount reaches the urine. When protein appears in significant amounts — proteinuria — it usually means the filtering units (glomeruli) or the tubules of the kidney are damaged or leaking. Unlike the albumin-creatinine ratio, which measures only albumin and is best for early detection, total urine protein captures all protein types and is used when larger amounts are expected or when following established kidney disease. It can be measured as a random 'spot' urine protein-creatinine ratio (which corrects for how dilute the sample is, giving a good estimate of daily loss from one sample) or as a full 24-hour urine collection. The amount matters: a small increase suggests mild kidney damage, while very heavy protein loss (in the nephrotic range) points to specific glomerular diseases and causes the blood albumin to fall, leading to swelling. Proteinuria is one of the two core measures of kidney disease alongside eGFR, and the two together are used to stage chronic kidney disease and gauge the risk of it progressing. A single positive can come from fever, exercise or infection, so persistent proteinuria is what carries weight.

What a high value can mean

  • Kidney (glomerular) damage — the commonest meaning; from diabetes, high blood pressure or glomerular disease.
  • Nephrotic-range proteinuria (very heavy loss) — points to specific glomerular diseases and causes swelling and a low blood albumin.
  • Tubular kidney damage — a different pattern of protein loss.
  • Transient proteinuria — fever, vigorous exercise, or urinary infection; usually resolves and needs repeating.
  • Overflow proteinuria — occasionally from excess abnormal proteins in the blood, as in multiple myeloma.

  • Normal (little or no protein) — the reassuring result; the kidney filters are intact.

Persistent protein in the urine is an important finding because it both signals kidney damage and, independently, raises the risk of that kidney disease progressing — so reducing it is a central aim of kidney care. The amount guides how concerning it is: heavy, nephrotic-range loss needs prompt specialist assessment to find the specific cause, while milder proteinuria is monitored and treated with blood-pressure control and kidney-protective medicines.
