Chest tightness

A feeling of pressure, squeezing or constriction in the chest. It has many causes — from anxiety, asthma and acid reflux to, importantly, the heart — and because a heart or lung emergency can feel this way, new, severe or exertional chest tightness must be taken seriously.

What it means

Chest tightness is a sensation of pressure, heaviness, squeezing or a band around the chest. It is a symptom with a wide range of causes, some minor and some serious, and telling them apart is the whole point. Common and often benign causes include anxiety and panic (a very frequent cause, often with rapid breathing and tingling), asthma and chest infections (with wheeze, cough or breathlessness), acid reflux, and muscle or rib strain (worse on movement or pressing the area). But chest tightness is also a classic way that heart problems present — angina (from narrowed heart arteries), typically brought on by exertion and eased by rest, and a heart attack, where it is more severe, persistent and often with other features. Serious lung causes include a blood clot in the lung (pulmonary embolism). Because of this overlap, the safe approach is that new, severe, exertional, or otherwise concerning chest tightness is treated as potentially serious until proven otherwise.

Common causes

  • Anxiety or panic attacks — very common; with fast breathing, palpitations, tingling.
  • Asthma or a chest infection — with wheeze, cough or breathlessness.
  • Acid reflux (GERD) — burning tightness, often after meals or lying down.
  • Muscle or rib strain (costochondritis) — worse on movement or pressing the chest.
  • Angina — heart-related; brought on by exertion or stress, eased by rest.
  • Heart attack — severe, persistent, often with breathlessness, sweating, arm/jaw pain — an emergency.
  • Pulmonary embolism (blood clot in the lung) — with sudden breathlessness — an emergency.

Lab work-up approach

Chest tightness is assessed urgently by its story and examination, an ECG (heart tracing), and, where a heart cause is possible, a troponin blood test to check for heart-muscle damage; where a lung clot is suspected, a D-dimer helps in lower-risk people, guided by clinical scoring. CRP and a full blood count support assessment of infection. These are done in an acute setting, not as routine outpatient tests, because the priority is to rule out the serious causes quickly.
