Anti-dsDNA antibodies
An antibody that is highly specific for lupus (SLE) and, unlike a positive ANA, rarely appears without it. Its level often rises and falls with disease activity, especially kidney involvement, so it helps both diagnose and monitor.
What it measures
Anti-dsDNA is an antibody directed against the body's own double-stranded DNA. It is one of the most specific blood tests for systemic lupus erythematosus (SLE): while a positive ANA is common and non-specific, a positive anti-dsDNA points strongly to lupus and is part of its classification criteria. It is present in roughly 60–70% of people with lupus, so a negative result does not exclude the disease. Its second value is in monitoring: in many patients the level rises when lupus is more active — particularly with kidney inflammation (lupus nephritis) — and falls with treatment, often mirrored by a fall and then recovery in complement (C3/C4). For that reason it is usually checked alongside complement and markers of kidney function.
What a high value can mean
- Systemic lupus erythematosus — a positive, and especially a high or rising, anti-dsDNA strongly supports lupus.
- Active lupus, particularly lupus nephritis — levels often climb with a flare, especially kidney involvement.
- Occasionally low-level positives — in other autoimmune conditions or, rarely, with certain drugs and infections.
What a low value can mean
- Negative / normal — reduces the likelihood of lupus but does not exclude it (30–40% of lupus is anti-dsDNA negative).
- In a treated lupus patient — a falling level can reflect a good response to treatment.
When to discuss with a doctor
Anti-dsDNA is checked when lupus is suspected — typically after a positive ANA with suggestive symptoms — because a positive result adds strong, specific evidence.
