Nucleated red blood cells (NRBC)

Immature red blood cells that still contain a nucleus. They normally stay in the bone marrow, so finding them in an adult's blood is abnormal — a sign the marrow is under heavy demand or stress, from severe anaemia, low oxygen, or a marrow problem.

What it measures

Red blood cells begin life in the bone marrow with a nucleus, which they normally shed just before entering the bloodstream — so a healthy adult's blood contains no nucleated red cells (NRBCs). When a modern blood-count analyser flags them, or a technician sees them on a blood film, it means immature red cells are being pushed out of the marrow prematurely, and that is always worth explaining. The commonest reasons are a strong demand for red cells that the marrow is racing to meet — severe anaemia, brisk bleeding, or breakdown of red cells (haemolysis) — or low oxygen states. More concerning causes are problems within the marrow itself or its infiltration: myelofibrosis, leukaemia, other cancers spreading to marrow, and severe illness or sepsis. NRBCs are counted per 100 white cells (and the analyser corrects the white-cell count for them, since it would otherwise miscount them as white cells). In newborns a small number is normal; in adults it is not. A rising NRBC count in a critically ill patient is also recognised as a marker of a worse outlook, reflecting the severity of the underlying stress.

  • Severe anaemia or brisk blood loss — the marrow races to produce red cells and releases them early.
  • Haemolysis — rapid red-cell breakdown driving high production.
  • Low oxygen states — chronic lung or heart disease.
  • Marrow problems or infiltration — myelofibrosis, leukaemia, or cancer spreading to the marrow.
  • Severe illness or sepsis — a rising count signals a sicker patient.
  • Normal in newborns — a small number is expected shortly after birth.

When to discuss with a doctor

Nucleated red cells should not appear in an adult's blood, so any that are reported deserve an explanation rather than reassurance. Sometimes the cause is already clear — a known severe anaemia or a haemolytic process working hard. When it is unexplained, or when NRBCs appear alongside other abnormal cells, an unusual white-cell picture, or teardrop-shaped red cells, it prompts a careful blood-film review and often referral to haematology to look for a marrow problem such as myelofibrosis or leukaemia.
