Vitamin E (tocopherol)

A fat-soluble antioxidant that protects cell membranes and supports the nervous system. Deficiency is uncommon and almost always caused by problems absorbing fat, in which case it can slowly damage the nerves — a treatable cause worth catching.

What it measures

Vitamin E (mainly alpha-tocopherol) is a fat-soluble antioxidant that protects cell membranes — including those of nerve and red blood cells — from oxidative damage. Because a normal diet supplies plenty and the body stores it, true deficiency is rare and almost never comes from diet alone. When it does occur, it is usually because fat is not being absorbed properly: chronic pancreatic or liver disease, cystic fibrosis, coeliac disease, or rare inherited disorders of fat transport. Prolonged deficiency mainly affects the nervous system, causing a slowly progressive problem with balance, coordination and sensation (a neuropathy and ataxia) — which is treatable if recognised, making it worth checking in the right context. Because vitamin E travels in the blood on fats, its level is interpreted relative to blood lipids (often as a ratio) rather than in isolation. High-dose vitamin E supplements are not clearly beneficial and, in large amounts, may carry some risk.

Vitamin E is a targeted, not routine, test.
