Bone-specific alkaline phosphatase
The bone-made portion of the enzyme alkaline phosphatase, released by bone-building cells. Measuring just the bone fraction gives a cleaner marker of bone formation than total ALP, and it is used especially to assess and monitor Paget's disease of bone.
What it measures
Alkaline phosphatase (ALP) is an enzyme found in several tissues — mainly the liver and bone. Total ALP therefore mixes bone and liver signals, and a raised total can come from either. Bone-specific alkaline phosphatase isolates the bone-derived fraction, produced by osteoblasts (bone-building cells) as they make new bone, giving a more specific marker of bone formation and overall bone turnover. Its clearest use is in Paget's disease of bone, a disorder of overactive, chaotic bone remodelling: bone-specific ALP is characteristically high, helps assess how active and extensive the disease is, and is followed to monitor treatment. It is also raised in osteomalacia (soft bones from poor mineralisation, often vitamin-D related), an overactive parathyroid, healing fractures, bone metastases, and normal childhood growth. Being bone-specific, it is useful when total ALP is raised but the liver enzymes (like GGT) are normal, pointing to a bone rather than liver source.
